Requirements
The University requires all program participants to submit a completed and signed Assumption of Risk and Release Form, an Emergency Contact Information form, and a Registration form. All three completed and signed documents must be included along with the registration deposit. Downloadable forms (in .pdf) are included below. The documents should be downloaded, printed, completed, signed and submitted along with the $250* deposit.
*Externship candidates should include an additional $25 processing fee.
