Registration
A $250* per person, non-refundable deposit must accompany your registration
form. A second payment of $500 is
due on March 1, 2013. Registrations
received after February 28, 2013 are contingent upon space availability and must
include both the deposit and second payment (total of $750) to reserve space.
The balance in full is due and
payable no later than April 15, 2013.
Due to the non-refundable nature of our accommodations arrangements and
host institution’s policies, cancellation between April 1 and May 1, 2013 will
incur a $500 per person penalty. If
cancellation occurs after May 1, there is a 100 percent penalty. The penalty may
be waived if a qualified substitute is found to fill the vacancy.
Acceptance is on a first-paid, first-reserved basis and subject to space
limitations. If your registration is
not accepted, the full deposit will be returned.
*Externship candidates must add an additional $75 processing fee.
A completed and signed Registration form, Assumption of Risk and Release form,
Emergency Contact Information form, Health Information form, a legible copy of
your current passport, and the $250 per person deposit must be submitted to
enroll in the program. Seats are filled on a first-paid, first-reserved
